PROCEDURE FOR FILLING THE APPLICATION FOR SCHEME (GROUP PERSONAL ACCIDENT INSURANCE FOR COIR WORKERS )

The claim application can be had from the Coir Board HO /Field Offices of the Board and also can be downloaded from the website www.coirboard.gov.in

Duly filled in claim form shall be submitted to the Field Offices of the Board. The Filed Officer in Charge after due verification have to recommend the claim form along with required documents to the Head Office. After due verification, the Head Office will forward the claim form to the Insurance Company for settlement

The following documents are required to be submitted along with application.

1 Death Certificate (in case of accidental death)
2 Police Report / FIR (wherever available)
3 Post Mortem Report (wherever available)
4 Identification of beneficiary, certificate issued by the CoirSociety/Unit/Establishment/ Nominated Officer of Coir Board
5 Recognized photo ID proof of beneficiary and nominee
6 Disability Certificate from a registered Medical Practitioner (in case of disability)
